We are happy to notify you that your bid, dated 25 March 2014, for the execution of LOT 4: CY 2014 AIRING OF DA RFO 1  RADIO PROGRAM, MONTHLY, AT THREE TIMES WEEKLY, 30 MINUTES PER DAY FROM APRIL 1, 2014 to MARCH 31, 2015 FOR THE PROVINCE OF ILOCOS SURwith ITB/Project Identification No. DA RFO 1-2014-GOODS-039, for a Contract Price equivalent to:

TWO HUNDRED SIXTEEN THOUSAND PESOS (PhP216,000.00)

as corrected and modified in accordance with the Instructions to Bidders, is hereby accepted and awarded you as the Single Calculated and Responsive Bid (SCRB).

You are therefore required to provide within ten (10) days the performance security in the form and amount stipulated in the Instruction to Bidders. Failure to enter into the said contract or provide the performance security shall constitute a sufficient ground for the cancellation of this award and the forfeiture of your bid security.
