We are happy to notify you that your bid, dated 16 December 2013, for the CY 2014 SUPPLY AND DELIVERY OF ONE (1) UNIT FLUORESCENT ANTIBODY (FA) MICROSCOPE WITH CAMERAwith project reference/identification number ITB No. DA RFO 1-2014-GOODS-022, for a Contract Price equivalent to:

ONE MILLION FOUR HUNDRED FORTY THOUSAND PESOS (PHP1,440,000.00)

as corrected and modified in accordance with the Instructions to Bidders, is hereby accepted and awarded to you as the Single Calculated Responsive Bid (SCRB).

You are, therefore, required to enter into contract with us and to provide the performance security in the form and amount stipulated in the Instruction to Bidders within ten (10) calendar days from receipt of this notice. Failure to enter into the said contract or provide the performance security shall constitute a sufficient ground for the cancellation of this award and the forfeiture of your bid security.
