About the Service

            All live animal traders, meat and meat product shippers are required to secure veterinary and health certificate and shipping permit. To facilitate availment of this service, our office located at Sevilla Norte, San Fernando City, La Union is open during working days the whole year round.

 

Who may avail of the service

 

Citizens who are engaged in the transport of live animals either as breeder, market or slaughter purposes and meat products passing through the region.

 

 

Requirements:

  1. BAI      LPHC license
  2. Veterinary      Health Certificate (from point of origin)
  3. Certificate      of ownership (if large animals)

Schedule of Availability of Service:

 

Monday – Friday: 8:00 AM – 5:00 PM

 

Schedule of Fees: 

            No fees to be paid.

Claim the VHC & shipping permit
