About the Service

The Issuance of Certificate for Land Use Reclassification is a pre-requisite in the processing and issuance   of Land Use Conversion Certificate by the Department of Agrarian Reform.

The following landholdings are covered by the land use reclassification:

  1. agricultural areas to be converted to residential, commercial, industrial, institutional and other non-agricultural purposes;
  2. areas devoted to agricultural activities such as livestock, poultry, and fishpond, the effect of which is to exempt the land from CARP coverage;
  3. areas converted to non-agricultural uses other than that previously authorized; and
  4. those reclassified to residential, commercial, industrial, or other non-agricultural uses on or after the effectivity of RA 6657 on June 15, 1988. For those reclassified prior to June 15, 1988, the guidelines of securing exemption clearance shall apply.
  5. Owners of private agricultural lands or other persons duly authorized by the landowner;
  6. Government agencies, including government-owned or controlled corporations, and local government units, which own agricultural land as their patrimonial property.

About the Service

            Plants, plant products and other materials intended to be exported undergo inspection and fumigation if necessary to prevent the harboring of pests and diseases in compliance with global standards.

 

Who may avail of the service

 

Any natural or juridical persons such as corporations, partnerships, associations, firms, companies and other legal entities.

 

 

Issuance of Veterinary Health Certificate and Shipping Permit

About the Service

            All live animal traders, meat and meat product shippers are required to secure veterinary and health certificate and shipping permit. To facilitate availment of this service, our office located at Sevilla Norte, San Fernando City, La Union is open during working days the whole year round.

 

Who may avail of the service

 

Citizens who are engaged in the transport of live animals either as breeder, market or slaughter purposes and meat products passing through the region.

About the Service

            All feed and veterinary drug and product outlets are required to secure a Bureau of Animal Industry (BAI) Registration Certificate of License to Operate. To facilitate registration and renewal of license, our office located at Aguila Road, San Fernando City, La Union is open during working days the whole year round to cater to the needs of licensees.

Who must avail of the service

Citizens currently engaged in animal feeds and veterinary drugs and products trading and those who plan to engage in said business within Region 1.

About the Service:

            All Livestock and Poultry Handlers and Carriers (LPHCs) are required to secure a Bureau of Animal Industry (BAI) Registration Certificate. To facilitate registration and renewal of license, our office located at Aguila Road, San Fernando City, La Union is open during working days the whole year round to cater to the needs of licensees.

Who must avail of the service 

Any person or business entity engaged in inter-provincial and/or inter-regional handling, transport, or distribution of livestock, poultry, hide and skin, eggs, manure and dung.

Requirements to be submitted

 

For Livestock & Poultry Handlers (LPH)

 

  1. Notarized      application form                                             
  2. Community      Tax Certificate                                                                     
  3. Tax      Identification Number (TIN)
  4. 2      pcs 1”x1” ID Photo

 

For Livestock & Poultry Carriers (LPC)

 

  1. Notarized      application form
  2. CTC
  3. TIN
  4. Pictures      of vehicle (front showing the plate number and side)
  5. Official Receipt of the Certificate of Registration of vehicle
  6. Deed      of sale (if applicant is not the owner of the vehicle or ownership has not      yet been transferred)
  7. In      case of partnership or corporation, the following documents are required:

 

    1. Photocopy of articles of corporation
    2. Photocopy of by-laws
    3. Photocopy of SEC or CDA registration
